Buying a house with my partner of 3 years and while we’re splitting everything down the middle with this purchase. Before I start calling around lawyers, is there not just a template I can purchase and fill out that just states we each are responsible for half, etc.? What is this type of contract called?

that is called a contract for disaster. Most purchases with “partners” ends up badly. You split up and there is no divorce court to help you, only civil court, which is expensive.
